In First Impressions George Freedman, DDS, gives readers a brief summary of products that have recently been introduced to dentistry, based on his clinical experience.

Just when you thought that lasers could not get more affordable, AMD Lasers has announced the Picasso Lite soft-tissue dental laser. Specifically developed for soft-tissue treatment procedures, the Picasso Lite replaces other modalities of soft-tissue treatment effectively and economically. While it has a maximum power of 2.5 W (less than the 7 W of its precursor, the Picasso), it provides all of the power that is needed for most routine procedures. Most gingivoplasty, troughing an d coagulation soft-tissue procedures can be accomplished in the 1-W power range. Surgical procedures for excising soft tissues typically require power in the 2.5-W range. The Picasso Lite cuts, excises, shapes, and coagulates soft tissues with greatly reduced trauma, irritation, and necrosis of surface layers. It is indicated for gingivectomy, gingivoplasty, frenectomy, exposing implants and unerupted teeth, and treating aphthous ulcers and herpetic lesions, among others. I. Introduction
As part of our commitment to advancing the adoption of laser technology in dentistry, AMD LASERS offers both Picasso and Picasso Lite diode dental lasers free of charge to select universities and dental schools around the world. AMD LASERS has established an educational program designed to ensure that state-of-the-art laser technology, training, and education is available to leading university programs and dental schools worldwide.
II. Program Outline
A. Eligibility Requirements
1. Must be an accredited university dental school program that demonstrates an existing or planned laser training education program.
B. Application Process
1. Download and complete the application form available at: www.amdlasers.com. E-mail completed form and signed program guidelines to: getmypicasso@amdlasers.com or fax to: (678)868-4108.
a. All new applications will be reviewed monthly. Award submissions will be determined based on: demonstrated need, ability to incorporate use of equipment into the dental laser curriculum, and creativity of the application essay.
b. Selected educational institutions will be notified in writing and will be awarded 1 (one) Picasso or Picasso Lite diode laser (at the discretion of AMD LASERS) along with a corresponding set of accessories at no cost to the university. Ordering additional accessories such as fibers or disposable tips will be the responsibility of the university.
c. AMD LASERS, at its discretion, may elect to conduct a training session with the department head and staff.
d. Applicants/applications not selected will be notified in writing and will be retained by AMD LASERS for a period of six months. Provided you have not heard from the company in the interim, you may reapply after that time has expired.
C. Terms and Conditions
1. Educational institution selected agrees to conduct a clinical study (within 24 months) using the Picasso/Picasso Lite, and share the results with AMD LASERS. The Company will reserve the right to publish and distribute the results of the study.
2. AMD LASERS requires that the department head is responsible for the security and maintenance of the Picasso laser. AMD LASERS also requires that the university return (at the university’s expense) the laser to the company for inspection/calibration after 12 months of use.
3. AMD LASERS reserves the right to cancel and modify the program at any time for any reason.
4. Educational institution selected agrees to allow AMD LASERS to use the university name in marketing and promotional materials.

Laser hygiene: It's the New Wave

Do you have a diode laser sitting in a corner of your office collecting dust? If so, get out the Endust and a dust cloth, and prepare yourself for some terrific soft tissue results.
In most states, hygienists are allowed to use lasers as part of their periodontal program, but tend to shy away from routine use. Even after a certification course, hygienists are not sure how to use the laser in a patient's mouth or incorporate the laser into their daily schedules. Common questions can include: How do I choose patients for laser use? How do I charge for the laser? What codes do I use?
Hygienists can use a soft-tissue laser in two ways. First, you can use it on every patient in your practice for bacterial reduction. The diode laser is set on a low wattage in a pulsed mode and without an initiated fiber (will not generate heat, only light). It is then used in the sulcus of every tooth in the mouth, prior to stirring up subgingival bacteria during the hygiene visit. This greatly reduces the chance for upper respiratory infections and cross contamination for both patients and providers.
Second, after the evaluation of the periodontal structures and debridement with ultrasonic scalers and hand instruments, you are ready for laser-assisted periodontal therapy (LAPT). The laser is set to a continuous mode and the fiber is initiated (creates heat). Any areas in the mouth with pocketing and inflammation will benefit from LAPT, which is a type of curettage.
The diode laser can differentiate between necrotic tissue and healthy tissue due to pigment and edema. The hygienist is able to selectively remove granulated tissue from the lining of the diseased sulcus with minimal anesthetic, bleeding, and discomfort.
How do you know when the pocket is clean? It's like baking a cake and checking with a toothpick to see if it's "done." If the fiber comes out goopy, it needs more time. If it comes out clean, it's done.
At this time, there are no specific codes for laser use, thus regular prophylaxis and periodontal fees should reflect laser use. Every patient can benefit from the diode laser during a hygiene visit, so selection is easy. It's up to you to explain these benefits and to integrate it into your schedule.
If you need help brushing up on your techniques, find a professional to help. If you just need to brush it off and use it in your daily routine, then I challenge you to do it. Your patients will really appreciate the long-term benefits. So give them the opportunity to praise your skills as "the laser hygienist," and you, too, will reap the rewards of healthier patients.
